How the wrist watches over the seventy-year-old heart
Hidden arrhythmias, quiet warnings, and the discipline of tracking a week — not a day — at a time.
Your chest starts to race — but by the time you reach the clinic, it has quietly settled again. As we age, these “hard-to-catch” arrhythmias grow more common. The small device on your wrist has changed that. It can be the witness at the moment the doctor never gets to see.
What the wrist can catch: hidden atrial fibrillation
The classic age-related arrhythmia is atrial fibrillation (AFib). It is also a leading cause of ischemic stroke, so catching it early matters. According to the Japanese Circulation Society, prevalence over age 80 is roughly 4.4% in men and 2.2% in women. This is not a rare condition.
The trouble with AFib is that early episodes come and go (paroxysmal). If the episode is brief, the ECG has often returned to normal by the time you reach a clinic — and the diagnosis is missed. That is precisely why a wrist device you can trigger at any moment is so valuable: it captures the moment that mattered.
The Apple Watch (Series 4 and later) records a 30-second single-lead ECG simply by resting a finger on the crown. Results are classified as “sinus rhythm,” “atrial fibrillation,” or inconclusive. In Apple’s clinical study, the specificity for sinus rhythm was 99.6% and the sensitivity for atrial fibrillation was 98.3% (Apple, official). Since May 2024, its “AFib History” feature — which quantifies the time spent in AFib — has also been available in Japan.
These are, however, tools for noticing, not for diagnosing. If a notification appears, do not self-interpret — take it to your physician. Conversely, symptoms can occur without triggering a notification.
Sleep and body, as numbers
A wearable also records sleep duration, resting heart rate, step count, and more. Watching your daily numbers, you begin to see your body’s “usual range.” On a day when your resting heart rate is higher than usual, for instance, that’s a signal to ease up.
The trick is not to react to any single day’s number. Read the week, not the day. This is the essence of health-tracking in the age of AI. The record is also material for your doctor. A vague “I feel a bit off lately” turns into a language a physician can act on.
This habit of owning your own data is the foundation of a longer healthspan. A clinic visits you a few times a year; a wearable stays with you every day. See your body as a line, not as isolated dots. That is the new normal for staying well.
How I use mine, at 72
Each morning, over coffee, I check the previous night’s sleep and heart rate at a glance. No elaborate settings. “The day after a good night’s sleep, I feel better” — I simply confirm that everyday truth in numbers. Not perfectionism; just what I can sustain. That is the discipline of the seventies.
A short checklist for choosing one
- ☐ Are the letters large enough to read comfortably with older eyes?
- ☐ Is charging simple? (A charging pad you just set the watch on helps you keep the habit.)
- ☐ Does it offer ECG and AFib notifications?
- ☐ Can it share records with family? (Useful for remote care.)
- ☐ Are the price and any monthly fee something you can sustain?
You don’t need the newest, most expensive model. The best device is the one you’ll actually wear every day. Which measurement would you most like to make visible first?
